Why is my form not working?

I put a jot form on my website. I have trialled it and am just stuck on 'please wait' after pressing  the button to submit the form. How can I correct this? I have had to pause a facebook ad because 3 ppl tried to use the fprm and it has not worked. 

I opened your webpage and I was able to replicate the issue when I made a test submission.

The solution is to re-embed the form using the Iframe method. This will prevent script conflicts between the webpage and the form. Here's a guide: Getting-the-form-iFrame-code
