we are missing a form submission

A Dependency/Delinquency form was submitted and a confirmation email was sent to the sender, but the form never came to us.  How can I retrieve it?

If a form was submitted successfully the data should be in with your submission data:

If it is not, let us know the email address that received confirmation and we will be happy to have a look to see what we can find.
