I can't not submit form for several days and it keeps stucking on "Please wait" error. Please help me check this error.

I've tried accessing the form you have linked to, but this form is disabled:
I have further inspected your forms and account, and I see it was automatically suspended by our Phishing Detector tool. This tool is in place to help us catch any forms that may be used against our terms, and it seems to have flagged your forms as possibly bad due to references to banks and lottery.
I have re-enabled your account now, so your forms are working again.
When it comes to why your form cannot be submitted when you click on the Submit button of a page, it seems to be because there are required fields on your form that remain unfilled on the other two pages of your form. The troubling fields are the two "Localized Calendar" widgets on the form.
Both of these widgets are set as required, and they require inputs to be made even if they are on other pages of your form. For this reason, I would recommend using our ordinary DateTime fields instead of the Localized Calendar widgets:

This is now resolved. Our developers have just released a fix that should sort this out.
Your forms should now be submitting normally. Kindly check and let us know if you see any other issues.
