I added my email. pretty sure all info is correct. However all test emails are coming from 'email@example.com' even tried selecting 'firstname.lastname@example.org but still comes from ...@jotform.com

The Test Email button is programmed to send an email that way. If you want to see the actual sender, you have to test the form live.
