form disabled

Hello all my forms say disabled and will not allow me to send them. Can you please help

I have checked your account and found that it was automatically suspended by our phishing filter. Upon further investigation, I found forms asking for a username and password. Please take note that collection of such sensitive information is a violation of our terms of use.

For the meantime, I have reactivated your account for you to be able to remove the said forms. A disregard of this will cause your account to be suspended again once it passes through the filter once more. So I recommend on doing this removal of the said forms as soon as possible.
