Required fields not showing on the submission form

    There are 177 submissions - on submission reference C&MR: 175
    Submission form received was on 26 Sep 2016 at 15:49

    There are important fields (which is set as required) are not showing on the notification email, why would this happen - as the submission reference C&MR:176 have the required field showing

    If required fields were not filled, the something may have caused the forms validation to fail.  I was unable to reproduce this issue with your embedded form, however, depending on the device used, the user may have been able to bypass the validation.

    Generally, replacing the script embed code with your forms iFrame code fixes any validation issues:
