Can i Create a form like this using jotform?

Hi,

I tried a lot to create a form like this. I have faced tons of problems while creating this.

1. The form background becomes page size and we cant resize the form height. It should be specific to width of the form.

2. Date fields became so tiny that the complete data was not visible and only calendar was visible over there. 

3. I was not able to put colored transparent background, it can only be transparent or solid color.

If I understand you correctly you are looking to create a custom form with the fields placed horizontally instead of vertical but are having some trouble.

Doing something of the sort can be a little tricky though we've had similar requests that were made possible by some of our other team members I know it's achievable but may require minor adjusting to get it just right.

For the most part you'll need to use changing the form width, field positioning, and the advanced form designer (or injected css) to make it work like that.

Please allow us some time and we'll look into this to see what we can help come up with for you and then we'll update you again as soon as we are able if anything.
