not collecting data from radio/image options - just empty fields

I cloned your form and I did a test submission. The selected answers are showing properly in the email notification. Here's a screenshot:

If the issue is still the same, please try deleting the current notification and creating a new one. Creating a new notification will generate all form fields automatically. Please check these guides:

email notifications are indeed working, the issue was that by export with xls sheet image options data were not collected. (the actual issue was lost by transfer from thread to thread)

Unfortunately, the images that you inserted inside the Radio Button field will not appear in the exported Excel file. However, you can create a Grid Listing Report or HTML Table Listing Report so that the image will show up. Here's a screenshot of the Grid Report:
