Unable to use a template

It wont let me click on either Use Template or Continue - when I do, nothing happens.

I want to use a basic contact form with captcha

Could you provide us with the link of the template that you are trying to use so that we can take a look and provide you with necessary assistance? Since you say, it is a basic form you may also create a new form by dragging and dropping the required fields in the form builder. Once you have all the necessary fields on the form, you may apply a theme from the theme-store. You may also design your own theme using the form designer. Please refer to the guide with basic video tutorials to use the form designer.
