    Are you referring to the account (draespinal)? As per checking the account draespinal, the account was automatically suspended because one of your form (http://www.jotform.co/form/62685025779872) contains a username and password field which is against our terms of use

     

    I have removed the fields on your form so I can activate your account again. Please avoid collecting sensitive information such as username and password to avoid your account from being automatically suspended again.
