Is there a way to create an online certificate to have populated with a respondents name at completion of survey?

    The completion of the survey is a requirement for the attendees to receive their continuing education credits.

    Once they complete the survey in it's entirity they would need to be allowed to print a certificate that includes t

    heir name for their files as proof of earning the education credits. Certificate is in the form of a template with the name being a merge field.

     

    Also is there a way to restrict who completes the survey via a list so only those attending will have access?  

     

    Currently, the survey is not completed but I can provide a copy of the certificate template if it would be helpful.

    You can actually attach a PDF copy of the submission in the Autoresponder email. The attached PDF contains the form field labels and the user's answers. The Autoresponder email is the confirmation email that your users will receive once they completed your form. Here's a guide: https://www.jotform.com/help/26-Creating-a-Form-Autoresponder.

    You need to enable the "PDF Attachment" option in the "Advanced" tab of the Autoresponder in order to attach a PDF copy in the email.

    You can customize the PDF using the PDF Designer. Here's a guide on how to open the PDF Designer:

    1. In My Forms, select the form and then click the "Submissions" button.

    2. Click the "PDF" button and then select the "Customize PDF" option.
