I'm an admin for a club page but Jotform doesn't give me the choice of adding a page tab

I'm an admin for a club page but Jotform doesn't give me the choice of adding a tab to the club page.

The "Add page tab" button is a bit misleading.  There needs to be a Facebook page tab already created in order to use this.  You will select the page tab that has already been created from the list of tabs shown then click on "Add page tab" which will add the form to the page tab you have selected.
