Form headers are missing from submissions

I am new to Jotform and I am trying to figure out when the student submits the form below how in my end I can print it with the header information. The way its set up is that I can only print the student information on what they fill out but the top half of the form is missing. Please help. Thank you in advance. 

If you are unable to see few fields in form submission or header is missing from submission, then please try to click the "setting" icon on submission report page, and then check all the form fields which you want to see on submission report. Also check the checkbox "Show Headers and Text", then you will be able to see complete data on submissions.
