"Cascading" drop down menues

Hi there,

I need to create a form for signups to my trainings.

As they are held on a specific date in a specific city, I need to create the following:

First the client chooses a city from the drop down menu. 

Then the client chooses a date from a dropdown menu AND the dates available of course depend on the first choice of the city.

 

Example: 

Let's say I have my training in NYC on Nov. 1 and Dec 1. and I have the training in L.A. on Nov 10 and Dec 10.

Then the first drop-down menu has the options "NYC" or "L.A."

If you choose "NYC" the date options that are shown must be Nov. 1 and Dec 1. If you choose "L.A." on the other hand the date options that are shown must be Nov. 10 and Dec 10.

The easiest way to do this would be to conditionally show different fields based on the selections made previously:

This would allow you to set up a dropdown for each set of dates for each city and show the appropriate dropdown depending on which city is selected.  The guide above shows how to do this with multiple fields, though it works the same for single fields.
