email@example.com is listed on the form, she in not listed on the email sent, so she is not getting the responses.
I have tried deleting and then adding her back, then saving but that did not change anything. I read that there is a limit of 10 recipients.

Try setting up a single notification for just that address to see if that will allow for it to send.
I checked our logs and did not see anything for that address so it looks like the notification is not triggering at all. If the new notification to only that address still does not send, let us know and we will be happy to have another look.
