Why some fields are not appearing in my form?

For some reason not all the fields are showing up on my form - only select ones.  I'm not sure if this is because of certain conditions that have been set but I've set the conditions as to how I need them. Also none of the graphics or form designing I did is showing up.  What's going wrong?

I have checked your form and I can see all the fields are appearing when previewing it, here is a screen-cast with the form opened in the Form Builder and loading the direct link:

Could you please confirm us that you can see the same now on your end? 

If this still persists, please provide us the field's name that does not appear when loading the direct link, we will be glad to further check your form.

 Also none of the graphics or form designing I did is showing up.  What's going wrong?

Are you referring that changes made in the Designer tool are not being applied? 

I have checked the current settings and I can see they are the default, there is not any injected code either. 

Could you please also provide us more details about this?
