The form works well, except that most of my fields (Quantity fields) keep chopping off the quantities in the numbered form.
For example, I have a dropdown box that has numbers 1 to 10. In all but the first dropdown box output on the form, the numbers don't show up when viewing the email.
I have tried modifying the HTML widths but my experience in this is limited and it doesn't appear to make any difference no matter what I change.

Have you tried resetting the email notification? Sometimes when the form is heavily edited and the Email Notification have not been recreated/refreshed, the changes made to form does not reflect in the email notification. Try deleting the email notification and redo by adding new again. This will then be populated by the fields in the form. Then check the email again by testing the form.

This User Guide article explains how to change the width of your form. I have cloned your form and did submission and it works fine, all fields submitted are showing. You can clone it and try, ensure you set your notification mail.

Deleted the email and recreated it. Now seems to work fine. I appreciate your help. Thanks to Gori as well.
