Unable to upload image

Hi,

I joined and created this form today.

Two of us have tested the form and we are getting error messages when attaching images to the form as the form asks.

Hello Scott,

I just tried attaching an image to your form and I did not encounter any problem so far.

If the problem persists on your end, can you provide us more information on the file size and all other relevant information of the image you are trying to upload?
