I think I might have created multiple accounts and only need one account. I have also lost my form: [embed]https://form.jotform.com/form/62666228620154[/embed], it's no longer showing up in my account. Please help. Thank you.

I did check your embedded form #62666228620154 and found that it is associated with account "cesclasses". The profile email address of this account is rob***@the*****associate.com.
Please sign in to your JotForm account with username "cesclasses" and you should be able to see your form.
