Need tags applied to Infusionsoft record when jotform is filled out

I integrated Infusionsoft with jotforms but don't see anywhere to apply a tag.  Can you help?

When you have completed the integration in your form you will be able to apply tags to the record generated, here is a screenshot of what you should see on your integration wizard: 

I have checked in your form and I can see tags are appearing, please mark those tags that you need to apply to the record once it is generated.
