When you try to use my form, it continually bumps the text back to the front of the line. I have been told by a few clients of this bug and I can only imagine how frustrating it is for them. Any ideas?Page URL:

We are sorry for what this issue has caused you.
I have opened your form with Firefox and made submission successfully with no problem and haven't seen the problem you stated above i.e ' it continually bumps the text back to the front of the line'. Would you please reproduce the issue your clients have? if so can you send us the screenshots of the problem
Can you try using a different browser and see if it is working fine.

Like my colleague, I'm trying my best to identify the problem. In which version of IE does the issue not show up? If I could compare what happens in that browser versus FF, I should (hopefully) be able to see the difference.
Annotated screenshots of what is displayed in IE vs FF may also help.

Nevermind. I just reproduced the problem in FF. I'll see if I can scour the internet for a solution.

Okay, my investigations so far have revealed that this issue doesn't show up when the form is viewed at its own URL. Moreover, I embedded the lighbox version of the form into a test page and the issue didn't show up there either.
My conclusion (at this juncture anyway) is that since your page contains jQuery, there's a conflict with with our form's Prototype script. Contrary to what we have recently been advising users, you could try appending a "2" to the lightbox's script (the version with the "2" on the end is a jQuery script) but to be honest, I doubt that will help.
I know you would prefer to have the form displayed in a lightbox but (assuming the append "2" solution doesn't work) perhaps it would be best to embed it via a pop-up until our developers can figure out what's wrong with the jQuery version of the lightbox script.

As suggested by my colleague, you should try add the "2" like this... the red 2.

I think that may have fixed it but I won't be able to tell for sure until I get home from work.

Tested it again. This is how it looks on my end.
Look like its fixed. =)
