I just opened an account and created a form. It says it is disabled. Please help!

Your forms and account was auto suspended by the Phishing detector due to your form that have fields that ask for account codes, security codes, username, password and the likes.
For now I have re-activated your account so that you can correct forms that have issue.
Please delete those fields on your form that resembles login/username/password/account so that the Phishing filter will not block and disable your account again.
