forms not navigating to custom url after submi

hi

just when i thought i had this under control

my form is not navigating to the pages indicated in the conditions

i did add this line

?institution={institution}&surgicalCase={surgicalCase}&attending={attending}&a_email={a_email}

as per what i had understood from jot support

the forms were working perfectly this morning,

but as i worked during the day, they no longer navigate

each form in the conditions seems to work great

but the navigation has stopped ....i cant figure

please advise

this is important to me to resolve soon

I have checked your JotForm and see that it is having only two fields while the redirecting URL is having 4 parameters. Could you provide us with the correct form URL and the options to be selected to replicate the issue at our end? This would help us in investigating the issue further and provide you with necessary assistance.
