FORMULÁRIO PARA AGENDAMENTO DE DATA E HORÁRIO

Gostaria de saber Como CRIAR UM Formulário para quê o Cliente possa agendar horários e datas para atendimento.

Este Formulário deverá ter vários horários e datas para a escolha do cliente para depois ele envie.
