Form not working for Mac user using firefox

Hey guys, we have reports of the following form having issues with Mac users on Firefox:

Some people can't upload their images... Some people click submit and nothing happens.

So you see anything wrong with the form? Is there anything you can suggest or test?

Really want to stay with you guys!!

Forms should now be working once again.  We had a brief server issues that has been resolved.  If any forms are still not able to complete submissions, let us know and we will be happy to have a look.
