Mobile version 'floating" on iPhone, but not Android

Why is it that the mobile version is floating on my iphone, but not my android? It's really strange, because it's not happening with all my forms on iphones, - just this one: http://www.napacitywinetours.com/inquire

With this form, it looks fine on an iPhone: http://www.downunderjourneys.com/inquire

What is the difference, and how can I make it stop floating?

I previewed your form on Iphone 5s but I was not able to replicate the same issue.  This is the screen shot of how it is seen on Iphone 5s. It is fit the screen and I was also able to fill or click the options on fields. 



In addition to that, I realised that mobile responsive widget has added to your form. I'm not sure if you added it more recently after sending this thread or it has already added even before sending this thread. Since, this widget is used to adjust your form to fit a mobile screen. 

Could you please give us some details and answer my question here above regarding mobile responsive widget.
