Have numbers displayed on my printed submitted forms?

    I love the design of the form, but find it hard to stay on track analyzing the submitted forms.  Is it possible to format the design of the form that is submitted?

    For example  1.  How many people do you let control you?

    Choose which applies best.

    a.

    b.

    c.

     

    The printed submission only displays the "choose which applies best and the number.  

    I would like to be able to see the number of the question on the printed submitted form.

    Please advise.

    You can view your submissions in the submissions page, from there, you can enable the "Show text and headers" option as mentioned in this guide: https://www.jotform.com/help/305-How-to-show-headers-or-text-for-contracts-terms-agreements-in-the-PDF-format-of-a-submission 

     

    The reason why you are seeing the "Choose which best applies" is because that's the actual label of the radio button field. The headers like "1. I try to be with people." are separate fields and are not directly related to the radio button field that you have as a question.

    These headers won't appear in the "Inbox" part of your dashboard, you need to have it directly viewed in your submissions page. 

    Or you can try editing your email template in your email wizard, you can see in your email template that the headers are not actually there, you can manually type in the questions for each radio button field listed in this data table to make it more organize and easily understandable when you receive it.
