My clients and I made multiple attempts submitting forms from different devices on different networks. Submission button changes from "Submit" to "Please Wait", but does not load. Refreshing page or backing out of form only deletes information. Forms are not being received to JotForm account as well.

Can you try updating the value of sandbox shown on the image above to "allow-forms allow-scripts"? Like this:
