If you are using full form code on your web site, you must update the code on your site.

To get the latest full form source code, open the form on the Form Builder:
    1. Open "Setup & Share" tab on toolbar,
    2. Click on "Share Form"
    3. Click on "Advanced Options"
    4. Click on "Full Source Code".

Thank you for this, but it is still not working. The form looks fine in preview, changes if I import the code, and importing the full source code doesn't make any difference, the form still changes. I have tried putting a link directly to the Jotform website but the form still changes. I am still building the website at the moment using Xara.

    Any suggestions please?

    Thank you

So, the Xara editor changes the form when you import it there.

One possible solution would be to use the IFrame version of the code. Here is how you can get it. Open your form on the Form Builder:
    1. Open "Setup & Share" tab on toolbar,
    2. Click on "Share Form"
    3. Click on "Advanced Options"
    4. Click on "iFrame Code".

Sorry, I have tried to import using all the methods you have suggested but the form always defaults to having questions following each other on different lines. I am trying to put two questions with check box answers side by side, as they are related. Do I give up and redesign the form?

I looked at your forms. They look fine.

I am not sure what you are trying to do but have you tried changing "Label Align" and "Label Size" from the Style toolbar?
