Hello!
I have an html document that is the width of a regular page. At the end of the questions, I want to add a yes or no box. Is there a way to do this?

I would suggest you to use "Radio button" field from the basic fields:

If I understood correctly, you want to show this document at the end, upon clicking on "yes" on the YES/NO question, correct?
To add the document you can use any of these widgets:

Then use a Show/Hide Conditional Logic, that when clicking on the Yes/NO radio button, you show the document,
