I have an html document that is the width of a regular page.  At the end of the questions, I want to add a yes or no box.  Is there a way to do this?

    Thank you very much!

I would suggest you to use "Radio button" field from the basic fields:

You can also verify it on my clone form here: http://jotform.com/62734558003959

If you have any query, or need further assistance, feel free to revert back.
