Hello… my client is having trouble when a form is submitted to them, that all of the fields are not coming through. It was working fine until a week or so ago… can someone please help solve this frustrating problem.

I am not sure if I have understood your question correctly. Do you mean to say that when form is submitted, all input data is not being displayed in your form's submission page?
I did test your form which is embedded in your webpage and it seems to work as expected. Form's submission page does display all input data correctly. Please check the screenshot below:

I would suggest you to please test your form again and get back to us if the issue persists.

The problem is the DATE doesn't show. Worked fine before now doesn't … ????

I think you meant the Matrix field in the form. This is because the field doesn't have a label causing an incorrect mail tags in the email body. I have corrected this for you. Please let us know if the issue persists.
