where can I find UNIQUE NAME and FIELD IDs for "thank you page"

    If you want to edit the source code of the Thank You Page, simply click on the source code icon:

    If you want to include answers, you can add the field from the "Form Fields" tab located in the Thank You page edit mode.

    Thanks, but I'm looking for the ID of the Thank You Page or its URL, if it exists at all.

     for tracking.

    If you are trying to use script on a Thank You page for tracking purposes like Google Adwords conversion code, that is only possible through custom URLs:
