You need to provide us the new username you would like to use, to check if the username you want to use is available, please go to the sign up page and enter the username there, you will know then if it is available:
https://www.jotform.com/signup
Once you have checked that the username you want to use is available, please let us know so we can proceed changing the username on our end.
