How do you create a table with rows and columns for choose?

Hi everyone!

I would like to create a basic table with 4 columns and 6 (or more) rows. 

There will be a heading for each row. 

The form is for choose only one equipament (in the example is the yellow line).

Then i will get the response by email (ex: 2150)

Unfortunately, this is not possible here in JotForm. However, we have tools and widget that might be useful for this.
