Why is my form not appearing in the templates?

My enquiry form is not appearing in the Form Templates.  I have shared my other form successfully but this one is not happening.

By the way, I suggest you provide more detailed description of your contact form so it will not be removed by the reviewer team. You can come up with a better form title too like "Slim Contact Form" or similar. Adding your company in the title is fine but a generic form title would be preferred since form templates is a public gallery of all the shared forms. It would help eliminate confusions when users understand the form content, description, and its title.
