It looks like you created a form using a guest account (guest_62759158742063). To collect the data on the form you created, I would suggest you sign up for a free account. Here's the link: www.jotform.com/signup. Make sure to use the same email address that you used as before.

Once you've signed up, you should be able to find the Puppy Application form you created in your new account. If the form is not on your account, let us know and we will move the form for you.
