Jotform link is not working.

    Representative Jared Polis’s office uses Jotform to receive and organize applications for Congressional nominations to a Military Service Academy.

    Last week, our office’s IP address was banned and our account suspended. As a result, our constituents weren't able to submit their applications. Today, it's come to my attention that the link isn't functioning again. The deadline for these nomination applications is fast approaching.

     If you are able, can you please assist our office in lifting the suspension on our account?

    I checked your links (link 1, link 2) and they seem to work successfully. I was able to see the forms on your webpage. I would like to inform you that your account is currently Active, so you should be able to use it without any issues.
