I have (conditional) required fields on my form and if they are hidden by the condition, the are causing a error only in IE. If the same form is used in Chrome, no issues.
This just started in the few past days... up this point it was working fine.

I have tested your form in IE and I can see the conditions are working fine, please see my screen-cast below about my test:
Could you please provide us more details about what issues are you experiencing on your end?
Please do let us know which version of IE you're using too, that will help us to properly test on our end, we will be glad to take other look to the form.

I have receive many calls from customers, it 100% is not working...
Because it was causing issues, I removed the required fields.. (made them not required...)
I tested it also before removing the required fields, and IE is a problem, so is firefox... Chrome was the only that worked...
So when you test (I can give you access, to make a field required...) let me know...
What is happening, they are picking the first service (surgical guide) and when they submit, they get the error...
How did you do the screen video? I will make one to show you...
So I turned on required on a couple of fields that I knew would be hidden based on the service selected (first one surgical guide)
Error on hidden field above (the way I got to was to change the service)....

Thank you for the detailed explanation. I cloned your form and set the Implant Details Table as required. I was able to replicate the issue on the cloned form. The Implant Details Table is hidden when I select "Surgical Guide", and I still get an error when I submit the form. The issue occurs both on Chrome and IE.
Please allow me some time to investigate the cause of this issue, and I will get back to you with my findings shortly.

After checking the form further, I discovered that the issue occurs when the Configurable List Widget is being hidden via a Form Collapse. When I select "Surgical Guide" as the service, the "Surgical Guide Design" form collapse is hidden, which means the Configurable List inside it is also hidden. This triggers the error that you encountered.
I will forward this issue to our backend team for further investigation. We will let you know on this thread once we have any updates.
In the meantime, I would suggest you add the Configurable List in your Show/Hide conditions to hide it instead of simply hiding it via the Form Collapse. By doing so, you will be able to submit the form successfully even when the Configurable List is hidden. Just so you know I have been using this "style" for the past 2.5 years... As of Friday 9/30, this error started to occur. Not sure if this helps...

Unfortunately we have not yet received any updates on this issue from our developers.
I cloned your form and did some tests there. I set the configurable list widget as required to see if they still show an error when I picked the first service.
So far in my tests, it looks like I am no longer getting an error when I pick the first service and submit, even when the Configurable List from the third service is set to required. I get redirected to the Thank You page successfully.
