    I have two instances of
the form pasted (one near the top; one near the bottom). Upon
completing the form, the 'thank you' message appear but it shoots me
back to the very top of the page at the header; this doesn't occur with
the bottom copy of the form.

Is this still an issue? I have submitted a test form entry, and since the form is located near the top, the thank you page position seems to be the same.

It still occurs.  The first "scroll frame" of my website (as seen on a 11inch Mac Book) is a large video.  Then the next section is a white colored 'what about us?' section; then a YouTube vidoe, THEN the (now) purple background-colored section wherein I placed this form.  Upon completing it, it jumps me back to the very top of the webpage where the video-header is (three sections prior).  Then I scroll down and indeed the 'Thank You' message is displayed (as per your screen shot), however, it shot me back up to the very top of the page's foremost top.  Please advise how to correct this.

    Hi Jeron,

I test the 1st embedded form https://form.jotform.com/62777266103155   (the one on top) on your website page, but I could not reproduce the issue as well.

See this video when I did the test...

Can you please take a screenshot image also on your end how it looks after you submitted the form. Perhaps we will be able to identify the cause of issue if we can check a visual on how it happen.

We will wait for your response.

    Thanks.
