Why are we getting a blank page each time we submit something into our jotform?

    So we use the jotform to create an upload artwork form on our site and have been using it for a very long time. It wasn't until recently did we receive complaints from our clients having the incapability of uploading pdf files to the form and no verification of their submission being sent anywhere. Once they hit submit, they are taken to a blank page. It didn't use to do this before. If you could, test out our form and see if you guys can pinpoint the problem it would be greatly appreciated.
