Creating a new form from an imported form

I'm trying to create a new form by importing a form already created. The form is located here: http://www.aabb.org/membership/governance/committees/trpdocs/AABB%20Jan%2018%202017%20Board%20Assessment.jpg 

However, when I try to import the document I receive an error message that the form can't be found. 

I am afraid that the form needs to be re-created manually. Our system cannot import forms from word or image documents.

The import tool can import other JotForm based forms, and some of the web forms only (it is not guaranteed that the form layout and fields will be copied identically).
