Other than adding a "Submit" button, publishing and posting the embed code on our site, is there something else I need to do to make my form live? I am not receiving notifications of submissions.

I can see one submission sent to your email address. This means that your form email settings are working. Please check your spam folder for the email. If it's there, make sure to mark it as not spam. The following guide should help you with regards to email issues:
