Why form embedded using the source code does not load properly?

Hello i have multiple forms in the app but recently when i download the source code of any of the forms the web page doesnt initialize correctly, i guess there is a maximum creation of forms or maybe i have skip something when downloading.

I can see you're using widgets in your form and you're also loading your form in a local file, please do note that some resources will require the webpage to be online in order to successfully load all the resources. 

I copied the source code of your form and embedded it in a local form, here is what I can see in the web console:
