    Our Lichfield Branch is trying to complete the incentive Jot Form on completion it shows a error message.

     

    There are errors on the form. Please fix them before continuing.

    Form effective from 11/01/16

    I’ve cloned and tested your form. I am able to reproduce the same issue that you're having.

    The issue is related to having a hidden field which doesn't have a default value. Hidden box fields can never be displayed in a form because they use the HTML tag "<input type="hidden">"  which, when interpreted by the browser is a field that's invisible to the user. 

     

    To resolve this issue you should either assign a default value to this hidden field or you should remove the required tag from the field. Because when it is both hidden and required at the same time it is impossible for the user to submit your form.
