    can your forms do the following?

     

    question 1:

    ___ response 1

    ___ response 2

    _3_ response 3

     

    question 2:

    _2_ response 1

    ___ response 2

    ___response 3

     

    the user should be able to enter a "1,2'or 3" in only 1 response field to each question.  the results need to be formatted to a csv or excel file for import.

    I looked through all the templates, but there isn't anything doing what I am looking for.  I need to do a survey with multiple choice answers, BUT need to enter a number in the response, not a checkbox, or a radio button.  I need each question to allow only 1 answer to be checked.  Do I use a text box input alongside each multiple choice answer and somehow tie them to each response?

    I think the closest option to your requirement is to use the Grading tool field . You can use this field to setup your question similar to what you described.
