Email subject line: is it possible to display a field from the form?

Hi 

I would like to set up custom emails that contain a unique identifier from the form that is displayed on the email subject line.  Is this possible?

Eg.  Form generator for {name} and {URN}

Yes, to do it first click on the email icon and on the edit button in the email notification: 

You will get the email wizard where you can customize the email template, in the subject line click on the pencil icon to edit it: 

You will get the form fields that you can add to the subject, once you have finished click on the apply button and save the changes:
