One of my forms is "Disabled" and "Can Not Be Found" - how do I "Enable" it?

    One of my forms is "Disabled" and "Can Not Be Found" - how do i "Enable" it?

     

    I clicked "Enable", but it still said it couldn't find the form because it was disabled (see attached screen grab)

     

    Also, the same form is NOT being forwarded to my email when someone completes the form. As a result I missed a couple of very important business leads because i didn't know they had filled out my JotForm form.

     

    Please help me get my JotForm form and it's email notifications working again.

    Your account appears to have been autosuspended by our phishing detector tool, which is why your forms were disabled. However, I checked your forms and they don't seem to violate any of jotform's terms. With that in mind, I have now removed the suspension on your account. You should now be able to use your forms without any issue.
