Is is possible to take photos in the form?

I need to create an app that can do these things:

1) Take pictures

Do you think this is possible?

Yes, seems to be possible to do and you will need to use some widgets to achieve it, I will provide each widget for each point required: 

1) Take pictures

This widget will help you to take photos in your form, you can also try the demo form in the widget's page: https://widgets.jotform.com/widget/take_photo 

2) Automatically store those pictures where I can access them remotely online

3) Automatically have the picture texted to a patient's  phone  (I was going to use Zapier for that part)
