How do I insert a chart with the option to change text fields?

I am trying o duplicate a year end employee evaluation sheet and I am needing to insert a chart where I can edit the permanent fields??

I don't think we have this feature. Can you give us an example of that chart? We can request it to our backend team, we just need to know its core function.
