A non-required field shows up as required

    Also, the tab that says shortage is not supposed to require a photo and it is currently.

    I failed to locate anything labeled as "shortage" on your List Damaged Item Details form.

    I checked all the Upload Fields on that form and confirmed ALL of them are required EXCEPT these 2:

    Are you still having issues or is this resolved? If possible, please give us more info or send us screenshots (here's how) that could better explain the issue you're reporting.

    It doesn't show until you actually enter the number of items you wish to claim. Then the boxes to select the problem with the merchandise come up.

    I have tried submitting the form http://www.jotform.us/form/50904593844159 and I was able to submit the form without any issue.

    I am working with a customer right this moment and the customer has marked shortage and it requires us to enter a photo

    When setting up conditions, you don't have to set the other side of the condition that wasn't met.

    e.g.

    IF Reason Code is Equal To Shortage THEN SHOW Upload Photo

    (this would suffice)

    You don't have to set it up like:

    IF Reason Code is Equal To Shortage THEN SHOW Upload Photo

    IF Reason Code is Not Equal To Shortage THEN HIDE Upload Photo

     

    When I checked your conditions pertaining toShortage, this is what I saw:

    Please re-setup your conditions in a way that you would only setup the showing of fields (don't setup the hiding part, as it's automatic if the condition isn't met).

    I would also recommend you have a look at some of our widgets that would make your form a bit tidier and simpler. There are widgets that allow multiple entries http://widgets.jotform.com/category/multiple_entries I would suggest you have a quick look at some of them in case you're interested.

    Jim I didn't set anything up =- it was set up for us. Is there a number I can call so someone can walk me through where to go to do all this?

    I have checked your JotForm now and tried to submit it. However, I was unable to submit the form. After further investigation, I found that there is a CSS code injected to your form which may be causing the issue. I have cloned your JotForm and tested after removing the custom CSS code injected and then tried submitting the form which worked fine.
